My heart breaks for the family who have endured this terrible tragedy.  Sometimes we just have to accept that God knows what is best for the lives of His children.  My 21 year old granddaughter was called home on June 23, 2007.  Her death was the result of a gunshot wound which the authorities ruled self-inflicted but since she was not alone when it happened, we too, have so many questions.  The thing that comforts me most is that the Bible tells us that God knew us before we were ever born and that He knew the days we would live on this earth.  So I know that she is safe with Him and that we'll miss her forever but we accept that He knew best.  May God bless each of you is my prayer. The "stuff" still goes on by the medical facility  / Tammy Reed (sister)

Tina, Even though the Imaging Center in Nevada, supposedly dropped me from the suit a year ago, I am still listed on all the court papers and documents because they never abided by the signed agreement, they drew up and presented to the court. Even though the judge signed to have me released, the lawyers for the Imaging Center continue to harass us. After they dropped their suit against the family, (right before it was to go to trial) because they couldn’t prove their case in court that they helped you, that they did the right things, that they had a license and that they were in the room with you, they dropped their suit and are pretending like it is no big deal that you went in for an appointment, were left alone, were found non-responsive when the technician returned at the end of your test, and they just all go on with their lives as if it was just another day at work.
Most don't work there anymore, others are facing other court battles for other "infractions" and some just went on to choose their next pair of shoes to match their evening gown. After all, that is what really matters...do my shoes match my dress?? Some got rehired and promoted for supporting what happened but in the end, they will all have to live with knowing what happened that day and that you are no longer with us.
You are not forgotten, you are remembered and we will continue to share your story with others in hopes of someone stepping up and wanting to correct the wrong. You can never come back to us, but the years of torment we had to endure because of what the facility did, then sued us and making us relieve that day for years, dropping the case because they couldn’t prove anything to the courts, still treating patients the same way you were treated because they saw no need to change anything, and still no justice for you. That was not the story of your life, but for now it is a chapter of your memories.
We miss you and will always remember you.

I just wanted to give condolences, although I dont know your family  I was on the site because I had made a tribute to my sister. She died on Christmas day from what we believe to be negligence of a hospital and doctors. We never got justice, and never even got to present to a court or get a lawyer to take the case. It is just am empty feeling of losing someone special, and to other people they was just a case. Its heartbreaking. So my heart and prayers go out to you, Tina and the whole family. I dont know you all but from one family to another in loss we know each other well. You have a wonderful page for your sister and keep up the good work of keeping her memory alive through those who still love her and always will. I hope it continue to bring you comfort. Dear Family of Tina Lockwood McCarley,

I am so sorry for your loss.  Watching Tina's slide show brought tears to my eyes.  What a beautiful person.  I too had a cat scan, and the first part went fine, but when they added the contrast drip, I was unable to speak, and a buzzing in my ears made me unable to hear what the nurse was saying.  I could only move a couple of fingers, and thankfully the attendant noticed, and stopped the drip.
I felt drunk and they got me off the table and offered me coffee.  I am not a drinker of alcohol or coffee.  I had to sit there for about 30 minutes.  The conclusion was that I was extremely allergic to Iodine, the main component in the contrast.  I wonder, hearing Tina's story, if by chance, through their neglect, they didn't see what she could not of said due to the reaction to the contrast.  I didn't want to unload all this on you, but it is so similar to my experience, I felt I had to respond.  I lived.  But a hospital in Nevada killed my dad with neglect, and we have no answers yet.  I also lived and watched my youngest daughter die of cancer, and every time they did a cat scan on her, the fear was there.  I will pray for you and her family tonight.

To Tina's Family, I was appalled to learn of the treatment the staff and doctors gave Tina on the day of her appointment at their facility. Worse yet is that there have been no changes. The fact that they then tried to sue you in court for talking about what happened when all you were doing was trying to get to the truth of what happened after she was left alone during her test. The courts of Nevada should be ashamed of themselves for letting the Nevada Facility drag you and your family thru the courts for almost 4 years and then when it came time to prove they "did right by Tina" they dropped suit and ran because they had no proof. The courts then dropped the case as if it never happened and failed to realize all the bills that the Nevada facility made you incur along with making you relive your pain daily because they wanted to inflict more hurt to more people besides the ones on their exam tables.
I read where even after being released from the suit and abiding to keep the full facility name out of the public eye, which you have abided to, that your name is still active and in the court documents a year later. I commend you and your family for again proving that you are the decent, honest ones even though the facility has cheated, lied, and deceived the courts thru all this.
I pray for you and your family and hope that 2008 brings honest, true and good things for you and your family. Tina's children are in our prayers. God Bless.

Well, it is almost Thanksgiving time and you are still in our daily thoughts. The Imaging Center had to drop the suit because they couldn't prove they did anything right and if they had faced the judge, they would have faced multiple fines for all the delays and time wasted. Their testimony will always be on paper now and we will always have that as proof what happened that day during your test.
The holidays are here again and another year passes. We continue to tell others about you and keep your memory alive. Until there is an apology to the family for the pain the facility has caused the family, we will continue to fight for the truth and share with others what occurred. Without justice for you from the Imaging Facility, there will never be peace in the family.

I followed up and checked on the doctors license as you suggested before my appointment and was appalled to find out that even after the death of your sister, now almost 4 years later, the doctor is still practicing without a valid license on file. I have contacted my doctor and requested that if he is going to refer me for tests, then I want someone who is trained, licensed and certified. I can only hope that your sister did not suffer, but hate to think of her lying there for almost 20 minutes alone during her test and the technician wasn't there as she was supposed to be. I noticed that the Imaging facility had other complaints on file also and am glad that I did my homework and am able to share your story with others. I am so sorry for your loss. It is sad that they took your sister by neglect, but that the courts then let them drag you thru the system, wasting time and money to prove they took care of your sister, but even after 3 year they could not prove to the courts or anyone else that they took proper care of your sister and they finally had to drop the suit. What chickens!!! I wonder how those people live with themselves every day knowing that they left Tina alone and neglected her medically and she died from it. For their lack of response, false testimony and no license, I hope that one day you are able to find justice for Tina and her children and put her to rest. I hope you and your family are able to find the peace and justice you all deserve for Tina.

Shocked / Diana Davis (Friend)

I was shocked to read that the doctors all the way to the head people refused to show a license when asked. Being a business person myself, your license is usually displayed out front so that people can see you are certified and legitimate. Even if it were not posted, if someone asked me to see my license, I would gladly and proudly show it.
To read that the doctor refused to show she had a license and she was supported by the staff and administration and they even put it in writing, saying "Do not show the doctors license" claiming that is personal and private information of the doctor's seems unethical.
I am so sorry that Tina had to die to expose the true horrors going on at this facility. The real injustice is that no policies or procedures were changed and they felt there was no need to do anything different.
I am sorry that they have not let Tina rest and kept her death alive for all this time. There is no peace, but by them dragging on their suit for so long and not proving anything, you now have their own depositions, their words, their carelessness and it is all documented and signed and court stamped. They cant deny it now as it was their lawsuit that put it all on paper.

Thank you so much for sharing your story with me. I live in Nevada and was being scheduled for a test soon. Your story made me more aware of what to look for when I am seeking a knowledgeable, trained doctor. I will look for a license, check with the BBB, and also will check the medical review board for complaints on that doctor or facility before I make my decision.
I too thought that because they were a "doctor or technician" that they were certified and schooled. Your story made me aware that is not always the case. I used to be scared going in for a test and having to sign the form that said I was aware "things" could occur but I always put my faith and trust in the person who had the lab coat on. Now I am no longer scared, because now it is much more than that.

I always thought that the person in medical field choose that career because they wanted to help others. After reading how the technician really felt about the patients and the doctors I was surprised to learn she was still in the field. Then again, the facility she worked at really didn't seem to care about standards or procedures so I can see how she was accepted for employment.

I am so sorry for your loss and the pain and suffering that the "Imaging Medical Facility" has caused you and your family. To have them drag you thru the courts for telling your story and then they cant prove anything says to me that you are telling the truth. I will continue to follow the stories in the sealed case and tell others what happened. Until there is justice for Tina and the family, this will never be over.

To the family, I am so sorry for your loss. After reading in the papers about what happened and how you cannot put Tina to rest, breaks my heart. I am scheduled to go in for a my own tests soon and this makes me think more about who I choose to see when the time comes. I used to just go pick anyone out of the phone book, but will now ask about licensing, training, and will make sure that I am seeing someone who I can really say I trust with my life. I just always thought the schooling and paperwork came with the title. 
Tinas story has made me more aware.

You were left alone during your test, and you died  / Tammy Reed (Sister)

Tina, today the "Medical Imaging center" (located in Nevada) that you went to, was finally supposed to prove they took care of you during your catscan, since they haven't proved anything in the past years since your death, but instead of proving they did anything right, they just dropped the suit against the family. They neglected you, left you alone, didn't perform CPR in a timely manner and after harassing the family by trying to sue us for asking questions, they still couldn't come forward before a court and say "We did anything right for this patient."
They took no responsibility for not being caring doctors, trained technicians, and walk away with no concern for the loss of your life.
They continue to treat patients in the same manner they treated you and for that I am sorry. I will continue to tell others about you and what happened and make them aware to check for legitimate licenses and to check references to make sure the staff is certified and not just given on the job training so they don't just blindly go to any facility no matter where it is located.
The technician, doctors and staff are probably laughing and celebrating that they got away with what happened to you and that they have not had to face any consequences for taking you away from us.
We will always remember you and what happened that day you went in for your rountine catscan and never walked out. Today there was no justice for you but you are not forgotten.

More Delays, More Time Wasted, Attorneys racking up fees  / Tammy Reed (sister)

Hello Tina, 
Another disappointment in the truth of what happened coming out. Another court date yesterday and the "medical Imaging center"  (located in Nevada) again told the Judge they still didn't have any proof to validate their case and the Judge gave them another extension. The suit against Mom and Dad continues and the lawyers for the medical Imaging center are hoping to drag this on and gain from your death from the family and get away with what they did (or didn't do) in this case. Again more months will be wasted before we can tell the truth and show others what really happened to you when you walked in to their office but never walked out. They go on with life and there has been no remorse, no justice, no changes, and no truth for you and the family. We should be able to remember you for the good times, but the facility and lawyers keep the pain and loss of your visit to them a daily reminder because they will not acknowledge what happened and end the suit. On each court visit, they seek more and more delays and are trying to drag this on for another year and soon it will be 4. The facility now is worried that if your medical records (that they had secretly sealed without the family's knowledge, authorization or signature) are unsealed by anyone that there is enough information in there to prove what happened (or didn't in this case) and so they feel that if they just keep delaying in court we will never have our time to speak out. It is a shame that you walked in for a routine appointment at their center and you never walked out, but they are able to walk away from any responsibility for leaving you alone, not assisting with CPR in a timely manner and so much else that resulted in your passing.

More months, more delays but one day everyone will read the notes, the transcripts and know that you were taken from us way to soon.
I will always wonder, had the technician been by you during your test, she might have noticed that you had stopped breathing or needed help. Instead there will always be that 20 minute period of your being alone before you were found non responsive. If she had been there during your test, maybe you would still be here with us today. We will never know, but will always wonder.

Still no justice, no remorse, no changes have been made and we wait till the day when the truth comes out.

Memorial Day is a time of remembrance and we remember you and Mike. While there were BBQ's and celebrations going on, those that you trusted were probably smiling and going on with their lives as if justice for your death just does not matter.
Mom and Dad are still being sued by the medical Imaging center (located in Nevada), for trying to find out what happened to you when you walked in for your routine catscan, but you never walked out of. From the reports given, we know You were alone for possibly up to 20 minutes before they found you non -- responsive and not breathing.
Now, 3 years later, they still have not even said "We are sorry for your loss," instead their reply has been an on-going lawsuit to gain financially for your death.
No changes have been noted that I know of to spare other family's, parents, sisters, brothers, and friends from the "same care" you received during your visit. To me it is like rolling the dice and hoping things come out OK, eventually someone will get the same hand you were dealt there. I feel sorry for those families when that day comes.

July 4th is coming up, a day of Independence. "We will not go quietly into the night! We will not vanish without a fight!" We're going to continue to fight for your life and always let others know you were taken from us long before you were ready!

One day we hope there is remorse and justice for your routine catscan that took your life.

Tina, I just read that in a 2006 interview in a public read paper in Nevada, the technichian named a facility she claimed she had worked at for over 16 years. I guess she forgot she had been employed elsewhere more recently and this is where you come in to see her for your visit and routine catscan. She continued on about her life, claiming, "It's the best time of my life! The experience in making new friends is irreplacable and once a *Sister, always a *Sister". 

After reading her interview, I have to agree that once a Sister, always a Sister and that experience is irreplacable. You should have been able to walk out of the medical Imaging facility instead you left in an ambulance and never woke up. 

As long as the case continues and and it is a daily reminder of what happended to you, we will continue to tell others, about your visit, where it was and what really happened. I continue to tell others your story and one day there will be justice for you.

Another month passes and still no corrections made in patient procedures  / Tammy Reed (Sister)
Hello Tina, another month passes and more delays in justice. The trial that was supposed to take place soon has been delayed by the attorneys for the medical facility. They now are dragging this on to a "maybe" court date of October 2007. Almost 6 more months from now. They are still sueing Mom and Dad and trying to find any reason or cause to have the family pay them for what happened, but how absurd is that. You go in for an appointment, a living, breathing person, and never to walk out of there and then you passed away soon after. It has now been over 3 years and we still fight for justice. There has been no change in policy or procedures at the facility that we have heard of so others still face the same things you did at your appointment. 
Christy is due to have her 3rd baby soon. Wish you could have been here to see your grandbabies. 
Heather's second baby, Evan has been in the hospital since birth and it may be another 2 months before he can even see his home for the first time. It has been touch and go for him since his "debut" but we know you are watching over him and helping him in his rough start. 
Keep watching over the family, especially the little ones, and we are still fighting to make a difference so you can one day find peace. Close

More Injustice  / Tammy Reed (sister)

Hello Tina. Well, here is the latest update on the family. The medical facility is still trying to sue Mom and Dad for asking questions about what happened during your test for those 20 minutes you were left alone and not found till the technician returned to finally check on you.
Even though I agreed to abide by their terms, the facility name was removed and I was released from the suit, they have chosen to re-add my name several months after the judge signed the papers and the court released me legally. The medical facility and its attorneys have ignored the court order and the papers they themselves drew up, and somehow choose to add my name back into the suit. (Without re-fileling to add me in)
As I have since I signed the papers, I have abided by the terms and will not, nor have I used their name. I guess when you have money to waste on a suit based on suing a family for asking questions about their daughter/sisters death, then you can afford to change the law to suit you and your client and not care about the consequences or effects.
So the harassment continues against the family because we dared to ask "What happened."
The facility still claims no wrong doing in leaving you alone, not assisting properly and right away and many other things. They continue to drag the family thru courts in hopes of winning money from the family for your death. It is a shame that no change was made to their policy, routine or procedures so others may face what you had to at your visit.
The trial is coming up soon, and since they have no intention of making amends for the tragic events that took place during your appointment, I assume we will all be in front of a jury soon. With all the evidence I have seen, it will be nice to finally be able to show the truth to the courts and jury, and let them know what all went wrong. Say Hello to the owners son who should be up there with you. His Dad filed suit against the college he was at because he fell off the dorm balcony. His dad is now trying to find justice in that sad event.
Maybe one day we will have justice for you.
Christy is doing well and it getting close to having her third baby soon. Little Evan was born 2 months early, but he is almost at a weight where Heather can bring him home soon. You would have been thrilled to be able to see your grandkids and have been able to play with them.
Well, keep an eye n the family. especially Mom and Dad. We all miss you. You are not forgotten.

Hi Tina, more months have passed and still no corrections have been made for what happened to you at your appointment. Besides Mom and Dad still being sued by the "Medical Imaging center" (located in Nevada) for talking about you, we still all wonder what happened during your test. I do not know if the truth will ever come out. Even though we have the documents from the employees, they may never get into the hands of someone that can make a difference and make your death not seem senseless. We are poorer for you absence in our hearts and we are also poorer for all the money it has cost us to defend ourselves and search for a reason. There has been no change in policy so others still face the same fate you did when you signed your name and put your trust into the hands of the staff and technichian that fateful day. 
Your death was senseless and being sued by the medical Imaging facility is even more senseless. You are with us always and we know you are watching over the whole situation. I hope that Heaven brings you the peace the family has not been allowed since you were taken from us.
