Memorial website in the memory of your loved one

This memorial website was created in the memory of my sister, Tina Lockwood McCarley who was born on Aug. 12 1959 and passed away on Jan. 26 2004 at the age of 44. We will remember her forever.

We all gathered for Christmas 2003, sharing in the wedding of our nephew. We gathered together celebrating, never knowing that in a few short weeks one of us would be gone and our lives changed forever.

Tina resided in Las Vegas with her family when she went into a Nevada Medical "Imaging Facility" to have a cat scan done on Jan. 12 2004. We know for the first part, Tina was OK. For the part with the dye, the technician who was performing the test, started this part of the procedure which took another 20 minutes. During that time, something happened. When test was over and the technician returned, she found Tina to be non responsive. An ambulance was sent and we waited for Tina to wake up, but it wasn't to be and the family made the heart wrenching decision to remove her from life support. Tina was moved to a hospice and in the early morning of Jan. 26 2004, God took her home.

Even though Tina is with God and at peace her family has not been able to find peace yet. We will never know what caused her to become non responsive during her test that day. While we do have our beliefs and opinions, Only God knows for sure.

My belief and opinions are that I know the technician did not like to deal with people that are hard to work with. She liked outpatient centers because people walk in, take their test and go home. She liked it better because she didn't have to deal with screaming doctors because they tend to get demanding. She did not prefer patients who were on a gurney, bedridden or needed help to the table. My sister was in a wheelchair due to a broken back from a hit and run, and falling several times at home. She would have needed help to the table, she would have been considered "hard to work with" to some people. We know Tina was responsive for the first part of the test because the technician had to keep reminding her to keep her arms up and still, which Tina did. For the second part, where the technician gave the injection, Tina was finally lying still and not moving her arms around, so the Technician proceeded with the films. When she returned to remove the IV 20 minutes later at the end of the test, it was at this time she finally noticed Tina was non responsive and not breathing. The technician went and got help from 3 other people before CPR was started in attempts to revive Tina, but by then attempts were unsuccessful. There is a 20 minute window of questions that the family will always have because what happened will never be known for sure.

My belief and opinions are that I know the "doctor" on duty was not trained in the procedures as to what the technician was doing, even though the doctor was supposed to be able to answer any questions if needed. In her opinion "She had her job and they had theirs." There were questions the technician had even before she began the test on my sister and she went to the "doctor on duty" for help and answers. I know the "doctor" was licensed under several names and refused to show she had a license (a decision supported by her colleagues) and continued to refuse to show she had one unless the family got a court order.

My belief and opinions are that I know the technician had "on the job training" before being placed in her position and there is no real schooling before you can inject someone with dye. I now know that when I go in for an appointment and I sign the form that says "are you aware death can occur," I used to have faith and believe that the person performing the test was trained and certified in a professional schooled manner. I never would have thought that it was just "on the job" training, but that thinking was all before this incident.

I have learned a lot in the past few years since Tina's death about right and wrong. I have learned that a building can be everything to someone while to most a human life is the most precious thing anyone can ever have in their lives. For the others, you may have money in the bank, but you may not have the love of family, friends or peace in your heart that most cherish.

The facility name has been removed and has been replaced with Nevada Medical "Imaging Facility." As opposed to using "building or office" as she was going in for a medical appointment scheduled by her doctor, "Imaging Facility" is just stating where she was and is in not in reference to any "Imaging Facility" in particular. After the "Imaging Facility" , located in Nevada, filed suit against myself (for having their name shown on this site) and my family, at this time I am not able to disclose why the name has been removed. Even though the name was removed, it was done so without answers, recourse for the actions taken by those there that day and the days following, without truth or peace for the family.

I can now say that the name was removed so that I would be removed from the suit. However, 2 months after my name was legally removed from the suit, signed and sealed by a judge, the Nevada Medical "Imaging Facility" choose to re-add my name to the suit, without a judges consent or authorization.

The Nevada Medical "Imaging Facility" also had gone in and had Tina's records sealed by a judge, so that we will never know what really happened. Tina's records were sealed without the family's knowledge, consent or authorization. We will always wonder what is in there that is so damaging to their facility that they had to quickly hide her records. If they had done their procedure correctly, if they had responded in a timely manner, if they were licensed, certified and trained properly as they claim, wouldn't an open file prove that?

After many attempts of the  Nevada Medical "Imaging Facility" repeatedly taking the family to court, and each time not being able to prove their case, delay after delay, it was at a point where the judge said "Prove your case or face sanctions". After almost 4 years of harassment to the family, the Imaging Medical Facility dropped their case as they could not prove that what the family was saying was false in anyway. I have read their depositions, office notes, and the  Nevada Medical "Imaging Facility" continues to practice in the same manner and treat patients just as they did when Tina went in. They felt her death was not reason to change procedure or to warrant keeping a technician in the room during a test. I will continue to let others know what happened to her and share her story.

Still today, there is no Justice for Tina.

Tina lived to be 44 years old. She is joined in Heaven by our brother Mike. We will always remember you both!

Our family chain is broken, and nothing seems the same, but as God calls us one by one the chain will link again.

Her legacy
December 2006  
Tina, Heather's daughter Ainsley will soon have a baby brother named Evan Lars Halvorson. He is due in the spring of 2007. 

Christy has Alexis and Michael and in the sping will add another little baby brother or sister to the family tree. 

You would be a very busy Grandma!

Updates  
Since Tina's passing, her daughter Christy has had two children. Alexis, who is now 1 1/2 and a son Michael who is now almost 6 months old. 

Heather, has had a beautiful daughter named Ainsley, and is expecting another child soon. 

Tina was not able to be here for any of their births.
Her Children  
Tina left behind two beautiful daughters Christy and Heather. Shortly after Tina passed away, Christy gave birth to Alexis Elizabeth Shaw. Tina was taken from us before she could see her first grandchild enter this world. Heather, who had joined the Navy, had to leave for bootcamp 8 days after saying goodbye to her mother.
